Are you highly organized and enjoy interacting with people?  Are you proficient in all MS Office products?  Do you want to work for an organization that makes an impact?   If so, we have an Administrative Assistant position available that supports our Community Service Programs.  You will provide administrative support such as answering calls, filing, creating documents, ordering and maintaining supplies, scheduling meetings, and data entry.  In this role, you will be integral to the continued success of our programs supporting individuals with developmental disabilities by assisting with state-required certifications and being the go-to person for the Community Services Director and Program Managers when they need assistance. We are looking for someone with a positive attitude, dependable, and who can maintain confidentiality.  You need to be flexible and have 2 years of similar experience.  High School Diploma or equivalent is required and you must have excellent verbal, written, and interpersonal skills.  Knowledge of the local communities is highly desirable and you must be willing to become a NH Notary.

Caregivers for Community Program – Newport/Claremont/Lebanon with daily regional travel

Full-time/Part-time, Hourly, Non-Exempt, Reports to Program Manager, $15 – $17 per hour depending upon experience, multiple positions available

Are you patient, reliable and passionate about helping others? Do you like to be active during the day and grab a coffee at DD, go bowling, volunteer at local organizations or walk around the track at the community center? Our Caregivers are sidekicks for individuals with developmental disabilities for participating in community and social activities. A typical day will start at 8 am getting your assignment for the day and then heading off to pick up your individuals in Sullivan County or Grafton County, grabbing a coffee and participating in an activity. You may have lunch with peers and their individuals and then on to an afternoon activity. Once you drop off your individuals in the afternoon, you complete daily paperwork. Your days are limited only by your imagination!  A High School diploma or equivalent is required as well as reliable transportation.  Excellent interpersonal skills and basic computer skills are necessary.  This role is a stepping-stone into a human services career.

Caregivers for Residential Programs – Lyme, Newport, Claremont

Full-time/Part-time, Hourly, Non-Exempt, Reports to Program Manager, $16 – $18 per hour depending upon experience, multiple positions available

Are you patient, understanding of people’s differences and attentive? Our Caregivers provide care and support for individuals with developmental disabilities and possibly behavioral challenges in a residential setting. At the residences, you assist individuals with daily living skills, safety, communication, and social skills as well as bathing, meals, activities, toileting and moving from location to location depending upon the individual.  You may also complete household tasks such as laundry, meal prep, etc. Each individual has at least one goal to work on and you will play a vital part in helping to achieve those goals.  Our residential programs provide a safe home environment and ensure the individuals participate and engage in their community.  You need to be a strong advocate for the individuals, a willing team player and motivated to create the best possible experience for the individuals you work with.  High School diploma or equivalent required.  A valid driver’s license, willingness to drive a passenger van and be medication administration certified is required.  Excellent interpersonal skills and basic computer skills necessary.

Early Childhood Educator – Claremont with Daily Regional Travel

Full-time, Hourly, Non-Exempt, Reports to FCESS Director, Hybrid work may be available after 90 days, $22 – $24 per hour depending upon experience

In our Family Centered Early Supports and Services program, the Early Childhood Educator position will provide developmental services for children under the age of three (3). Responsibilities includes both assessment of delays in areas such as motor, communication, cognition, social/emotional and self-help skills as well providing direct support and service coordination for children and their families. In this role, you will maintain direct service for a designated number of children, writing, and keeping current case notes and files as well as participation in weekly team meetings and all scheduled in-service transdisciplinary meetings.  You will participate in transdisciplinary assessment and evaluation of designated children using a specified assessment tool and act as the liaison between children, families, and local community agencies that provide supports for families.  Coordinates 6-month reviews, annual evaluations and new IFSP’s for all assigned children. Secures all necessary releases and consents as well as updated medical information and family changes that may affect the child’s development. For the position, an Associate’s or Bachelor’s degree in child development, early childhood education, psychology, social work, or other related field required.  You will bring functional knowledge from his/her profession about support strategies and treatment techniques considered to be best practice for children birth through age two and have specific training and experience in child development.  You must be proficient in MSOffice programs and demonstrate effective verbal, interpersonal and written communication skills.  You must be able to work a flexible schedule to meet Agency and family needs and have a valid driver’s license, proof of valid personal auto insurance and access to reliable transportation.  You will work both in an office setting and in family homes to provide services.

Employment Support Professional – Lebanon with Regional Travel

Full-time, Hourly, Non-Exempt, Reports to Program Manager, Hybrid work may be available after 90 days, $15 – $17 per hour depending upon experience

As an Employment Specialist/Job Coach, you act as an advocate for and support to individuals with developmental disabilities in obtaining employment. You work with individuals in a variety of employment environments and on a variety of employment tasks including applying and interviewing as well as learning and adhering to employers’ policies and procedures. You cross-train with your team members as well as coach employers and other community members on how to provide quality supports for individuals. Your main location is our Lebanon office; however, most of your days are spent at various community and employer settings within Sullivan and Lower Grafton counties in New Hampshire — mileage paid for travel.  A HS Diploma or GED is required and an associate’s degree in human services or related field is highly preferred.  You must have or be willing to take ACRE (Association of Community Rehabilitation Educators) training.  Previous human services, developmental disabilities or human resources experience is a plus and strong problem-solving skills a must.

Employer Liaison – Claremont with Regional Travel

Full-time, Hourly, Non-Exempt, Reports to Director of Individualized Services, Hybrid work may be available after 90 days, $21 – $25 per hour depending upon experience

Are you especially skilled at supervising employees who working varying schedules? Are you very organized and adept at problem solving?  Our Individualized Services department supports children and adults with developmental disabilities and their families to be actively engaged – with the Employer Liaison’s assistance – in supervising the employees that work with them on a daily basis. The liaison facilitates recruitment and hiring, coordinates required training for employees, works through performance management dilemmas as well as appropriate manager training for families.  Bachelor’s degree preferred.  1+ years of similar experience required. Additional years of relevant experience may be substituted for a degree.  Proven relationship-building skills; strong communication skills – verbal, written, and interpersonal – required.  Ability to travel regionally for job fairs, meetings, interviews and maintain a flexible work schedule required.  Basic knowledge of various employment laws and practices necessary. MS Office required with ability to adapt to new software and systems is necessary.

*NEW* Family Services Administrative Coordinator – Claremont

Full-time, Hourly, Non-Exempt, Reports to Area Agency Operations Officer, $17 – $20 per hour depending upon experience

As the Family Services Administrative Coordinator (FSAC), you will provide administrative and clerical support for the Family Services team, which includes PDMS, Family Services, Family Support and Family Centered Early Supports and Services.  Your responsibilities will include though are not limited to data entry into various internal and web-based systems, uploading documents into program specific files, and organizing and maintaining department files.  You will also collect, track and maintain database information  as needed for reporting needs, assist with the RFP (Request for Proposal) process, type notes for departmental staff meetings and create/modify documents as needed.  You must have at least a high school diploma or equivalent; an Associate’s degree is preferred.  You must also possess 2 – 3 years of administrative support experience and be very proficient in MSOffice applications.  You must have strong interpersonal, written and verbal communication skills and be highly organized and detail-oriented.  Familiarity with Case Management functions is highly desired.

Family Services Coordinator – Claremont with Regional Travel

Full-time, Hourly, Non-Exempt, Reports to Director of Family Services, Hybrid work may be available after 90 days, $21 – $23 per hour depending upon experience, Two (2) positions available

Do you possess excellent organizational skills?  Do you have a couple of years of case management experience?  If so, our Family Services Coordinator position may be for you.  In this role, you will assist individuals with developmental disabilities and their families by providing service coordination and advocacy while linking them to existing community and state resources. Here are a few examples of your responsibilities: You advocate and support families and individuals in a manner that increases opportunities for participation in the community by responding to the changing needs of the family.  You coordinate assessments and evaluations for all individuals as assigned.  You secure all necessary referrals, documentation, and relevant data to determine the individual’s unique needs including though not limited to the Support Intensity Scale (SIS) and Health Risk Screening tool (HRST). You facilitate planning meetings for individuals including completing, compiling, and processing Individual Service Agreements/Amendments/Action Plans and other related documents as requested or required. You participate in an ON-CALL rotation as scheduled. In other words, you serve as the hub in the wheel of an individual’s and their families’ support with PathWays. 2+ years Case Management experience is necessary.  Strong MSOffice and general technology skills needed.  Excellent communication and interpersonal skills necessary.

Individualized Services Supervisor – Claremont with Regional Travel

Full-time, Hourly, Non-Exempt, Reports to Director of Individualized Services, Hybrid work may be available after 90 days, $22 – $25 per hour depending upon experience

Our Individualized Service Team works with individuals with developmental disabilities and their families in optimizing choice and self-determination.  If that idea appeals to you, do you have experience with Case Management?  Are you looking for the next step in your career?  If so, then our Individualized Services Supervisor role may be for you.  You will provide direct supervision to the Case Managers in the department including performance management, team meetings, etc.  You will also review, edit and approve Individual Service Agreements for the individuals and families in the program.  In addition, you will conduct quality assurance checks for compliance with state and federal guidelines and provide case list coverage and backup for the Director as requested.  Bachelor’s degree in human services or a related field is preferred though strong experience can replace the educational requirement.  A minimum of two years of experience in program management and two years of prior case management experience is required.  Strong verbal, written, and interpersonal communication skills are necessary.  A minimum of intermediate proficiency with MS Office and willingness to learn new software is necessary.

LPN – Lyme, NH with Regional Travel

Full-time, Hourly, Non-Exempt, Reports to Program Director, $25 – $30 per hour depending upon experience

Are you patient, understanding of people’s differences, compassionate and attentive? Do you want to work for an organization that makes an impact?  If so, working as an LPN in a residential setting for individuals with development disabilities may be for you.  You will give meds, monitor health care needs, interact with physician’s offices, conduct QA reviews and provide medical information for various reporting requirements. Successful completion of an LPN program is required as well as an active registration and license to practice as an LPN in NH required.  1+ years of recent licensed LPN experience required and 2+ years’ experience in Human Services highly desired.  Must be able to work a flexible schedule and at varied locations (if needed) to meet agency needs.

Mainly Part time (Hours per week range from 5 to 40 depending on the position), Hourly, Non-Exempt, Reports to Employer Liaison, Most positions pay $14 – $16 per hour with some higher wages based on the needs of the individual, multiple positions available

Would you like to make a difference in the life of a person with developmental disabilities?  Are you reliable, patient and attentive?  If so, working 1:1 with an individual with development disabilities to provide guidance and support as needed in the areas of safety, daily living, communication, community, social and leisure skills may be for you. Current opportunities in Enfield, Hanover, Lebanon, Goshen, Newport and Claremont. Good verbal and written communication skills as well as basic math and computer skills (MSOffice) required.  Excellent communication and interpersonal skills necessary.  Must be able and willing to work a flexible schedule between varied locations as needed.

Program Manager for Community Programs – Newport with Local Travel

Full-time, Hourly, Non-Exempt, Reports to Director of Community Services, Hybrid work may be available after 90 days, $22 – $25 per hour depending upon experience

We are looking for a team player to oversee the daily operations of a community program ensuring quality services are provided to the individuals served. Flexible thinking and a knack and desire for problem-solving are key to supervising direct support professionals and serving as the liaison for teams and meetings related to the services provided. You will need to perform direct care as needed and may be assigned to various locations based on program needs. You will know you have been successful in this role if after a year you have made strong connections with the families/guardians/home providers of the individuals served; made your team better through positive contributions; and, helped others at PathWays by assisting individuals make progress on their goals, contributed to the development of staff, assisted other managers as needed, or developed the program.

Residential Program Manager – Lyme with some Regional Travel

Full-time, Hourly, Non-Exempt, Reports to Program Director, $23 – $26 per hour depending upon experience

We are looking for a team player to oversee the daily operations, in conjunction with another manager and nursing staff, of a residential home ensuring quality services are provided to the individuals served.  You will supervise residential support professionals and serve as the liaison for teams and meetings related to the services provided.  You will need to perform direct care (including personal care such as toileting, bathing, feeding, etc.) as needed as well as ensuring that daily tasks such as laundry, cooking, etc. are completed. You will also engage in enhancing and/or developing community resources and oversee records management for the program. Candidates must be able to provide at least two professional references and Bachelor’s degree in a related field is preferred — extensive program management experience may substitute for the degree.  You must have minimum of 3 years of supervisory experience and at least 2 years of human services experience; developmental disabilities and/or residential experience is highly preferred. Excellent verbal, interpersonal, and written communication skills and at least intermediate proficiency in MSOffice is necessary.  You must possess the willingness/ability to travel within the region/state as needed and therefore a valid driver’s license, proof of auto insurance and access to a reliable vehicle are necessary.

RN/Nurse Trainer – Claremont, NH with Regional Travel

Full-time, Salaried, Exempt, Reports to Senior Director of Community Services, Hybrid work may be available after 90 days, $2,960 – $3,100 bi-weekly depending upon experience

Are you patient, understanding of people’s differences, compassionate and attentive? Do you want to work for an organization that makes an impact?  If so, working as a RN/Nurse Trainer with individuals with development disabilities may be for you.  You will assess and monitor health care needs, conduct nursing assessments and provide medical information for various reporting requirements. You will conduct in-service training and workshops for staff regarding health and medical issues and certify staff in medication administration in accordance with applicable regulations.  An Associate’s degree in nursing required. Active registration and license to practice as a RN in NH required.  Minimum of 2+ years of recent licensed RN experience required and 2+ years’ experience in Human Services highly desired.   Nurse Trainer certification is highly desired or you must be willing to obtain it during your first six months of employment.  Must be able to work a flexible schedule and at varied locations to meet agency needs.

Support Broker (Case Manager) – Claremont with Regional Travel

Full-time, Hourly, Non-Exempt, Reports to Director of Individualized Services, Hybrid work may be available after 90 days, $21 – $23 per hour depending upon experience

Join us as a Support Broker in our Individualized Services group and support individuals with developmental disabilities.  You will carry a caseload of 25 to 30 individuals.  To be successful in the role, you must have excellent time management skills, the ability to frequently and efficiently switch gears between tasks, and strong interpersonal skills. Yes, this is a challenging role; however, it is always worth it to make the connections with individuals and their families and to see their growth.  After one year, you will know you were successful if you possess a solid understanding of the Individual Service Agreement process; you have established a fluid schedule of visits and team meetings with your individuals and families; and, you have a comprehensive understanding of the needs and circumstances of the individuals and families you serve.  Bachelor’s degree in related field is highly preferred; High School diploma or GED required.  2 to 3-year minimum case management experience.  Extensive case management experience may substitute for degree. Minimum of 2 years’ human services experience a must; experience with developmental disabilities preferred.  Excellent verbal, interpersonal, and written communication skills required. Intermediate proficiency in MSOffice (Word, Excel, PowerPoint) and ability/willingness to learn new programs a must.
