Current Employment Opportunities as of February 15, 2017

 Direct Support and Outreach Providers:   Full time/Part Time.  Day, Evening, Weekend and overnight shifts available in Residential and Day programs in Newport, Claremont, Lebanon and Lyme, NH.   Your responsibilities may include:  assisting people with activities such as household chores, recreational activities, learning activities, grocery shopping, laundry, physical fitness and money management; assisting or performing aspects of personal care such as brushing teeth, toileting, bathing, clothing and feeding; completing daily communication logs, progress notes, accident reports; and/or providing safe transportation for medical appointments or community activities. Good verbal and written English communication skills as well as basic math and computer skills (MSOffice) required.  Interested in assisting individuals achieve personal outcomes.  Must be able and willing to work a flexible schedule and at varied locations as needed.

Job Coach/Direct Support Provider:          Full time – time split between Job coach and DSP responsibilities.  Acts as an advocate for and support to individuals with developmental disabilities in a variety of employment tasks and in a variety of employment environments, including adherence to employers’ policies and procedures. Cross-trains co-workers and coaches employers and other community members on how to provide quality supports for individuals.  Education, special training, or experience in human services/public service preferred. Ability to maintain a positive, professional demeanor necessary.  Available for flexible weekly schedules that accommodate individual and program needs required.  Excellent oral, written and interpersonal communication skills.  Creative thinker with strong problem solving skills a must. (See Direct Support Provider description above)

Family Service Coordinator:           Full time.  Based in Claremont, NH with regional travel.  Assists individuals (primarily adults) by providing service coordination and advocacy while linking them to existing community and state resources.    Assists individuals with budget administration and serves as liaison with community, state agencies and service providers.  Bachelor’s degree in human services and case management experience including IEP’s, high school transitions, and future planning required.  Extensive case management experience may be substituted for a degree.  Minimum of 2 years human services experience and excellent communications skills: verbal, written and interpersonal required.  Strong writing skills – writing sample will be requested.  Ability to multi-task and prioritize as needed; Budgeting experience highly preferred.

Support Broker:        Full time.  Based in Claremont, NH with regional travel.  Works in partnership with individuals, and those they choose to support them, acting as liaison in designing, implementing and monitoring customized, responsive supports that promote individual choice and decision-making authority. Facilitates and/or coordinates interview and hiring process for support providers, and provides supervision for assigned employees. Maintains documentation, required data and records. Assists individuals with budget administration. Serves as a liaison with community, state agencies and service providers.  Bachelor’s degree and two years human service experience, with supervisory and budgeting experience.

 LPN/RN:         Full time and Part time at our Total Care Residence in Lyme NH. Associates Degree in Nursing (LPN), active NH license, 2 years licensed nursing experience within the last 5 years, and First Aid/ CPR. Ten hour shifts are available which may be flexible for full time.

Nurse Trainer:   Full time. Based in Claremont, NH. Provides health care services to individuals receiving services. Provides direct nursing care at our Lyme, NH residence one shift per week.  Assesses and monitors client health care needs and makes appropriate referrals. Conducts nursing assessments including preparation of care plans.  Conducts monthly home care quality assurance visits. Conducts in-service training and workshops for staff and Home Providers regarding health and medical issues. Participates in nursing On-Call rotation.  Associate’s degree in Nursing required; Bachelor’s degree preferred. Active NH RN license required. 2+ years of recent licensed nursing experience with a minimum of 1 year as an RN required. 2+ years in Human Services highly desired. Completion of Bureau of Developmental Services (BDS) Nurse Trainer course or ability to complete it within 60 days of hire a must.

 Nurse Supervisor:    Full time. Based in Lyme, NH.  Opportunity to innovate and develop quality supports in a leadership role. Oversees and supervises nursing staff providing care for up to 12 individuals in a total care residence.   Conducts nursing assessments and care plans, serves as liaison with health care practitioners, and ensures a safe medical environment for consumers and staff. Candidate will be degreed and NH-licensed RN with supervisory and leadership experience and professional level communications skills essential. They must have 2 years licensed nursing experience within the last five years, with at least one year as an RN.

Personal Mentors:    Part time.  Works 1:1 with individuals with development disabilities to provide guidance and support as needed in the areas of safety, daily living, communication, community, social and leisure skills. Supports individuals to participate in activities and access resources that will assist them in achieving their personal goals.  Some service provision may occur in the individual’s home or at a worksite. Opportunities are throughout lower Grafton and Sullivan counties.  Good verbal and written English communication skills as well as basic math and computer skills (MSOffice) required.  Interested in assisting individuals achieve personal outcomes.  Must be able and willing to work a flexible schedule and at varied locations as needed.

Quality Assurance Manager:          Full time.  Positions available in Lebanon and Lyme, NH.  Serves as liaison for work groups and focuses on quality assurance.  Models and assures advocacy and support to individuals, working to expand community opportunities.  Supervises direct support staff.  BS in human services or related or Community Social Services Certificate preferred.  4+ years human services experience.   2+ years supervisory experience a must.  Willing to perform direct care as needed and work at any assigned location as needed.   Excellent communications skills.  Position requires flexible hours and on-call assignments.

Early Supports and Services Provider:     Full time.  Based in Claremont, NH with regional travel.  Provides developmental services for children under the age of 3. The position includes both assessment of delays in areas such as motor skills, communication, cognition, social/emotional behaviors and self-help as well as providing direct support and services coordination for children and their families. Services are provided in-home for families. The ESSE works in cooperation and consultation with other team members to support developmental growth of the child.  Bachelor’s degree in Early Childhood Development and have or eligible to obtain a NH license in Early Childhood Education. A degree in Special Education or Elementary education will be considered with licensure eligibility. Psychology and Social Services degrees will only be considered with early childhood experience. Valid driver’s license and access to reliable vehicle is required. Excellent communication skills — both written and verbal are required as well as strong interpersonal skills dealing with both young children and adults is a must.   Position requires flexible hours.

Contract Speech Pathologist – Contract.  Works with children aged 0-3. The position provides both service coordination and direct services including conducting evaluations; in addition, the person will work in collaboration with local school districts and other area resources and be a member of a trans disciplinary team. Services are provided in-home. This position requires NH licensing and Certification and experience with children aged 0-3 highly preferred.
