OUR SERVICES: Creating Individualized Pathways

PathWays works with children, adults, and their families to provide ongoing, individualized support.

PathWays works with local agencies, programs and businesses to provide individuals with meaningful work, recreation and community life.

Family Centered Early Supports & Services – provides home based therapeutic services for children birth to 3 who have a developmental delay or are at risk for a delay. Our services include information and referral, service coordination, and educational advocacy.

Family Support – provides supports for families and children and young adults ages 3 -21, and for adults who still live with their families. Our services include information and referral, educational advocacy, in-home supports, and respite.  

Service Coordination – provides supports for adults with developmental disabilities or brain injuries. Our services include coordination of needed services, advocacy, information and referral, and in home assistance to maintain an independent lifestyle.

Employment Options – Work is one way for each of us to learn, grow and add value to our lives. The Employment Options team helps people realize their goal of being successfully employed in a job of their choice. In addition to providing income, a successful work experience often leads to the development of meaningful and lasting relationships. Click for an Employment Options flyer.

Day/Employment Services – provides day and vocational supports for adults with developmental disabilities or brain injuries. Our services include employment, volunteer opportunities, and community based activities and experiences.

Residential Services – provides residential services for adults. Our services include in-home supports, adult foster care, individualized supports, and small group arrangements.

Advocacy and Advisory: Partnership and Support

Our Family Support Councils serve in an advisory capacity to PathWays. One is based in Claremont and one is based in Lebanon. The Councils are made up entirely of family members who meet monthly, providing advice from the family perspective.

The Upbeat Group and the Upper Valley Neighbors are self-advocacy groups that meet monthly. Self advocates discuss common goals and work with PathWays on special projects that impact individuals receiving supports throughout the region.
